Intensive Community and Home Services
OVERVIEW
Intensive Community and Home Services (ICHS) provides an intensive treatment program for boys and girls ages 6-12 yrs with severe behavioural disorders such as aggression and defiance. The program is informed by research and incorporates CDI’s SNAP® model. Treatment is a partnership of family members and ICHS staff who jointly set and evaluate service goals. The program focuses on each family member learning and practicing more effctive ways of functioning and interacting at home and in their community.
PROGRAM COMPONENTS:
Each family is assigned a primary ICHS staff who works with them in the following ways:
Home and Community:
· Home-based support and coaching
· Building connections and use of community resources
· Collaboration with schools to support student success
· Advocacy and community support
At CDI Facility:
· Weekly parent and child groups
· Access to family and individual counseling
· Weekday summer activity program
· Section 23 classroom for students who are unable to function in community schools
Treatment Components:
· Anger management
· Cognitive-behavioural therapy
· Social skill development
· SNAP® approach (self control and problem solving strategies)
· SNAP® Parenting
· Using community resources to develop positive recreation and creative outlets/skills
Home based meetings with ICHS staff allow caregivers to learn to address the needs of their children through using parenting skills that work! Working in the environments in which the problems are experienced provides the best support for developing realistic and caregiver – directed approaches. Ongoing treatment evaluation is done in partnership with caregivers in order to promote positive relationships that can maximize the probability of positive long-term outcomes. Supported transitions to other programs and school placements, follow-up visits, continued access to family counseling and parenting groups are available after ending active involvement with the home-community based services.
REFERRAL TO ICHS
Clients are referred by professionals such as school social workers, teachers, children’s aid societies or other agencies and all referrals are processed through Centralized Access to Residential Services (CARS). The average length of treatment averages 6-12 months, but is determined on an individual client basis. Funding is provided by the Ontario Ministry of Community and Social Services and the Ministry of Child and Youth Services. There is no cost to clients.
